I understand that Cell On Wheelz is not an authorized service dealer for Apple, Samsung or HTC and that repairs rendered by Cell On Wheelz LLC may void manufacturers warranties for this device. Cell On Wheelz LLC does not assume any liability or warranty in the event that the manufacturer warranties are voided.

I understand that Cell On Wheelz LLC offers no verbal or written warranty, either expressed or implied, regarding the success of this repair and I expressly waive all claims against Cell On Wheelz for any damages to this device or data that is lost due to the repair rendered by Cell On Wheelz LLC.

I fully understand that I, the owner of this device, have backed up all information on the hard disk in which I want to preserve. I also undertstand that Cell On Wheelz LLC is no responsible for any data lost, and waive any legal action against this company. I understand that the data on this device is not the responsibility of Cell On Wheelz, and may be lost during the repair process without my prior knowledge.

I understand that Cell On Wheelz LLC is not responsible for items not claimed within calendar days of the date listed below. I also understand that Cell On Wheelz LLC will recycle or sell any device not picked up after 14 days from the date below regardless if the item was repaired or not. If left after 14 days it will be considered abandoned and will become the property of Cell On Wheelz LLC.

I understand that as a result of submitting my personal device for repair that it could be subject to loss, theft, damage, and data loss.

Our Technicians will not browse through your hard drive or memory cards looking at your data; however, they may inadvertently see data during the course of their repair. Please remove any personal or private files you do not want others to see.

Customers agree to resolve all disputes with Cell On Wheelz LLC through binding arbitration. By signing the repair authorization, you waive the right to a lawsuit.

SIGNING CONFIRMS THAT YOU HAVE READ, UNDERSTAND, AND AGREE TO BE BOUND BY THESE TERMS AND CONDITIONS.

*IF UNABLE TO PERFORM A PRE-CHECKLIST WE DO NOT ASSUME RESPONSIBILITY FOR ADDITIONAL FUNCTIONS NOT WORKING PROPERLY AFTER THE REPAIR. ANY FUNCTIONS NOT WORKING PROPERLY AFTER THE REPAIR ON DEVICES THAT WE CANNOT PREFORM A PRE-CHECKLIST ON ARE NOT COVERED UNDER THE REPAIR AND WILL ONLY BE FIXED AT THE EXPENSE OF THE CUSTOMER. I ALSO AGREE TO THE $25 DIAGNOSTIC FEE CHARGE IF PHONE NEEDS TO BE INSPECTED. I UNDERSTAND THE $25 DIAGNOSTIC FEE WILL BE PUT TOWARD THE FULL COST OF THE REPAIR IF THE DEVICE IS REPAIR WITH CELL ON WHEELZ LLC.
